5. Special Protections for Substance Use Disorder (SUD) Records

Some health information related to substance use disorder (SUD) treatment may be protected by federal law (42 CFR Part 2) and California law.

- These records receive extra privacy protections.

- In most cases, we cannot disclose SUD-related records without your specific written authorization, even for purposes that are otherwise allowed under HIPAA.

- When permitted by law, SUD-related information may be used or disclosed for:

  - Treatment

  - Payment

  - Healthcare operations

  - Public health and safety purposes

  - As otherwise allowed or required by law

Important: Any disclosure of SUD-related information is subject to stricter legal limits, and recipients may be prohibited from redisclosing that information without your permission or as allowed by law.

Other Uses and Disclosures

We may also contact you for:

- Appointment reminders

- Treatment alternatives

- Health-related benefits or services

We will not use or share your information for marketing or sell your information without your written authorization.

Any other use or disclosure not described in this Notice will be made only with your written permission. You may revoke your authorization at any time in writing.

Your Rights Regarding Your Health Information

You have the right to:

- Get a copy of your health records

- Request corrections to your records

- Request restrictions on certain uses or disclosures

- Request confidential communications

- Get a list of certain disclosures we have made

- Get a paper copy of this Notice at any time

- Choose someone to act for you

- File a complaint if you believe your privacy rights have been violated
